Many serious accidents have occurred because the machine was running when it was supposed to shut off for repairs or maintenance. These incidents are typically caused by the energy being not released properly. Lock-out-tag-out (LOTO) which makes sure that machines are off until a person with authorization releases them, helps to prevent these incidents.

The LOTO system consists of a set of locks tags, locks and other devices that are used to secure, isolate or block the device from electrical power. All "affected", "authorized" and "qualified" personnel should be instructed in the use of lockout/tagout devices. These devices include, but are not limited, to locks, chains and padlocks. Each device is singularly identified and labeled, containing the date and time of the application, the name of the person who is authorized to use it, and the reason for the lockout/tag-out (repair maintenance, repair, etc.). It should be stated clearly that these devices are not able to be removed until a complete LOTO procedure has been completed.
